Jeremi Piamonte, MSN, FNP-C, BC

Jeremi Piamonte is a dual board-certified Family Nurse Practitioner (AANP, ANCC) with over seven years of advanced practice and two decades in healthcare. His diverse background includes Emergency Medicine, Urgent Care, Primary Care, and Occupational Health, shaping a multidisciplinary and compassionate approach to patient care. He completed a fellowship in Pain Management for Primary Care at UC Davis, gaining advanced skills in diagnosing and treating acute and chronic pain with techniques such as trigger point and joint injections, cryotherapy, and other non-opioid therapies. His focus is on safe, sustainable, and individualized pain management. Jeremi has practiced in both high-acuity and community settings, including Carbon Health and several Federally Qualified Health Centers across Southern California. Known for his strong clinical judgment and patient-centered care, he is dedicated to improving outcomes and empowering patients through education. Fluent in English and conversant in Tagalog, Jeremi is proud to serve diverse communities throughout Southern California.
